Our proposal submission requirements changed on January 1, 2010. We no longer accept the Colorado Common Grant Application.

Government entities, schools, school districts and universities should contact the grants manager regarding specific requirements for the narrative and attachments.

Those requesting capital grants exceeding $10,000 must first submit an introduction letter prior to being invited to submit a full proposal. Click here for capital project requirements.

A. Cover Letter – one page tailored to address the interests and specific priorities of the funding source and amount requested.

B. Summary of Applicant Organization: Both the board chair and executive director or main contact should sign and date the Summary of Applicant Organization. It should include:

1. Contact Information

a. Organization name and address
b. Name of executive director or main contact person with job title
c. Phone, fax, email and web addresses

2. Purpose of Grant – (choose one):

a. General operating (support for the organization as a whole)
b. Special program/project (specific activity of the organization consistent with its mission)
c. Capital expenditure (funds spent for additions or improvements to plant or equipment)
d. Start-up costs
e. Endowment (Please note: generally, Rose Community Foundation does not make grants to endowments.)
f. Technical assistance
g. Other

3. Brief Description of Request
As part of the description, also list fiscal year end and amount of request

C. Narrative – include the following information in any order (preferred length not to exceed three pages):

1. Agency Information

a. Mission statement, brief statement of organization's goals and/or objectives
b. Brief summary of organization's history
c. Description of current programs, activities and accomplishments

2. Purpose of Grant - this section should include the following:

a. Brief statement of the issue to be addressed; description of constituency served (include number served); target population; how they will benefit
b. Description of goals and objectives for the purpose of the grant
c. Description of activities planned to accomplish these goals - whether this is a new or ongoing activity on the part of the sponsoring organization
d. Timetable for implementation (if funds are for a specific program or capital project)
e. Other organizations, if any, participating in the activity
f. Long-term sources/strategies for funding at end of grant period

3. Evaluation – please discuss:

a. Expected results during the funding period
b. How you would define and measure success
c. How the project's results will be used and/or disseminated

D. Attachments – please attach the following:

1. Board of Directors

a. List of directors, occupations and/or community affiliations
b. Antidiscrimination statement adopted by board

2. List of names and qualifications of key staff

3. Most recent fiscal-year-end financial statements (audited if available)

4. Current agency budget (include list of pending and promised foundation support)

5. Program/project budget (if applicable) including:

a. Program name (not applicable for general operating expenses)

i. Itemize budget line items and costs
ii. Total cost of program

b. Funds available for program – gifts and grants pledged or paid

i. Trustees
ii. Corporations
iii. Foundations
iv. Individuals
v. Government
vi. Other (earned income, special events membership, subscriptions, etc.)
vii. Total funds available

c. Balance required (total cost minus total funds available)

6. Budget Narrative - describe significant line items in your budget. if you have questions about what to include in this narrative, please click here.

7. A copy of the most recent IRS determination letter, issued within the last five years, indicating 501(c) (3) or 509(a) tax-exempt status

8. List of major contributors (and amounts) to organization/program (if applicable)

9. List of volunteer involvement and in-kind contributions
